Recommendations for PROMIS® pediatric measures in children and adolescents with chronic pain: A COSMIN systematic review of measurement properties

Patient-Reported Outcome Measurement Information System (PROMIS®) pediatric measures were recently developed to assess the multidimensional health and well-being of children and adolescents. Measures were developed with input from youth, caregivers, and experts, with items selected using advanced techniques. However, the measurement properties of these measures in pediatric populations with chronic pain have not been systematically examined. We conducted a systematic review applying the COnsensus based Standards for the selection of health Measurement INstruments (COSMIN) methodology to evaluate PROMIS® pediatric measures in children and adolescents with chronic pain. We evaluated 9 measurement properties (e.g., content validity, reliability, responsiveness) for all self-reported PROMIS® pediatric measures in all existing versions (item bank, short-form, and computer adaptive testing) from 64 studies. Recommendations were made for the use of each measure in both pediatric chronic pain clinical trials and non-clinical trial studies. For example, we found 4 short-form PROMIS® pediatric measures were recommended for use in pediatric chronic pain clinical trials, 9 approached recommendation, and 11 would be recommended with further evidence. Findings will be discussed in the context of pediatric chronic pain clinical trials and practice, and recommendations made for further evaluation of the PROMIS® pediatric measures in children and adolescents with chronic pain.
