Abstract Description
Dr. Kwon will describe the development of a novel AYA Healthcare Transition Clinic for emerging adults with chronic pain. He will discuss types of patients referred to the clinic, evaluation content and format, and goals of the clinic. He will present key learnings from the team’s experience running this innovative program.
