Augmented brain responses to multisensory stimulation in affective/regulation circuits is associated with worse quality of life in adolescents with chronic musculoskeletal pain -TU47

Abstract Description

Chronic musculoskeletal pain affects 30-50% of adolescents, with girls reporting more pain than boys. Chronic musculoskeletal pain can impact school attendance and physical activity, as well as quality of life. This study aims to investigate the relationship between brain responses to multisensory stimulation and self-reported quality of life in adolescents with chronic musculoskeletal pain. We included N=129 (110 females and 19 males), adolescents (12 to 18 years, mean age = 15.58, SD = 1.57) with Chronic musculoskeletal pain (recruited from three sites: Stanford University, Cincinnati Children's Hospital, and Toronto Sick Kids). An fMRI multisensory task was used to study the relationship between brain responses to non-painful multisensory stimulation and self-reported quality of life. We found a negative correlation between task-evoked activation in the rostral ACC and the dmPFC and quality of life. Also, lower self-reported psychosocial quality of life was associated with higher unpleasantness ratings during the multisensory task. These findings suggest that augmented processing of non-painful sensory input —typically present in daily life—in affective/regulation circuits is associated with experiencing low quality of life in adolescents suffering from chronic musculoskeletal pain.
